Registration

In order to register, the following items must be turned in:
 | Registration form filled out in full. |
| Personal history form filled out. |
| Enrollment fee of $30 per child or $50 per family. |
| Immunization record complete and signed at the bottom. |
| Emergency card with emergency contact people and date of last DPT. |
| Health report (signed by a physician) due within one month. |
